Sample Collection & Mailing Instructions

Please follow the instructions below carefully to ensure accurate processing of your sample.

ACCEPTABLE SAMPLE TYPES

You may submit one or both of the following:

• Nail Clippings
• Hair Sample

Nail clippings are the standard sample used within this protocol. Hair may be submitted as an alternative or in addition if desired. Liquid samples are not accepted.

OPTION 1: NAIL CLIPPINGS

  1. Wash hands or feet thoroughly before clipping.

  2. Do not apply lotion or oils prior to collection.

  3. Clip small pieces from at least 2–3 nails.

  4. Place clippings in a small baggie and seal.

  5. Label clearly with:

    • Full Name

    • Date of Birth

    • Date of Collection

OPTION 2: HAIR SAMPLE

  1. Wash hair with normal shampoo only (no conditioner, styling products, or oils afterward).

  2. Allow hair to dry completely.

  3. Cut a small amount of hair from the nape of the neck (close to the scalp if possible).

  4. Approximately 10–20 strands are sufficient and do not need to be longer than 1/2 inch in length.

  5. Place the hair in a small clean baggie and seal.

  6. Label clearly with:

    • Full Name

    • Date of Birth

    • Date of Collection

IMPORTANT NOTES

• You may submit hair, nail clippings, or both together in the same labeled baggie.

• Ensure samples are clean and completely dry before sealing.

• Only one individual per envelope.

• Samples are used solely for informational bioenergetic assessment and are not analyzed for medical, chemical, or diagnostic purposes.

• Don’t forget to complete and submit your Intake Form.

MAILING INSTRUCTIONS

Place your sealed, labeled baggies inside a standard mailing envelope and send to:

Standard mail is sufficient. Tracking is optional.

TURNAROUND TIME

Qest4 Initial Assessment reports are typically delivered within 1–2 business days after your sample is received.

You will receive:
• Baseline Evaluation Report
• Comprehensive Evaluation Report

If you choose to move forward with a follow-up consultation or support plan, additional preparation time may be required prior to your scheduled session.
